This crucial reversal period is key for high-risk groups, including South Asians, to regain normal blood sugar levels. Most people think pre-diabetes creeps up slowly, but doctors now say it comes with a hidden countdown clock. Miss that window, and your chance of stopping type 2 diabetes slips away. Catch it early, though, and the body can bounce back far better than most imagine. New research and expert opinions are now putting a spotlight on this crucial reversal period, urging people—especially high-risk groups—to act sooner rather than later. An important Indian study published in The Journal of Diabetes & Metabolic Disorders has revealed something many don’t expect: people who take charge of their pre-diabetes within the first two years of diagnosis stand a strong chance of halting its march toward type 2 diabetes. The researchers highlight this early stretch as a game-changing opportunity, where lifestyle changes have their best shot at pushing blood sugar back to safer levels. Jack Ogden, a primary care physician specializing in diabetes at The Lagom Clinic in Bristol, United Kingdom, told Medical News Today that testing usually begins around age 35. However, he pointed out that certain groups should be screened earlier. For higher-risk communities, including South Asians, he explained that testing from around age 25 may be more appropriate. Most people learn about their status through a blood glucose assessment, such as the HbA1c test. Ogden emphasized how quickly lifestyle changes need to come into the picture. He said that preventing the slide into type 2 diabetes involves aiming for a 5–10 percent weight loss, spending at least 150 minutes a week on physical activity, and focusing on a diet built around vegetables, whole grains, and lean protein. He also stressed the importance of quality sleep, reducing stress, and quitting smoking, noting that these habits can shift the trajectory early on. He added that the study reinforces how the first two to three years after a pre-diabetes diagnosis act as the most effective window for reversal. Despite access to healthcare, around 30 percent of people with pre-diabetes still progress to type 2 diabetes within a decade. Yet up to 60 percent manage to return to normal blood sugar levels when they intervene early, which he said highlights how targeted steps taken at the right time can pay off in significant ways. Apollo doctor, Dr. Sudhir Kumar, shared a similar message on X a couple of days back, explaining that pre-diabetes is most reversible within those initial two years. He noted that many individuals reach the pre-diabetes stage before developing full-blown diabetes, and this early phase offers a real chance to pull back to normal ranges. His advice centers on four pillars: lowering overall carbohydrate intake, combining aerobic exercise with strength training, losing weight for anyone who is overweight or obese, and maintaining healthy sleep patterns. A decade-long study titled 'Transition between prediabetic and diabetic stages in an urban community in India: A decade-long retrospective cohort study' adds even more depth to this picture. Researchers observed 1,670 adults aged 30 and above over ten years, tracking whether people stayed in normal blood sugar ranges, moved to pre-diabetes, or developed type 2 diabetes. They calculated separate incidence rates based on where participants started. The findings were striking. Overall, the incidence of type 2 diabetes was 20.94 per 1,000 person-years. Those already in the pre-diabetes stage faced a much higher rate of 41.74, compared to 15.89 for individuals who began with normal blood sugar levels. Their analysis showed that roughly 30 percent of people with pre-diabetes progressed to diabetes, while about 25 percent of those with normal levels moved into the pre-diabetic range. The standout discovery was that nearly 60 percent of people moved from pre-diabetes back to normal ranges within the first two to three years. After that, the likelihood of reversal steadily dropped. The study also noted that men were more likely to progress to diabetes, while women had a slightly better chance of returning to normoglycemia.
